Instructions: For Continuing Education credits there is an additional fee of $200 per workshops week, and a fee of $30 is required for each transcript. Please make checks and credit cards payable to Italy Art Workshops. Print and mail this form completed with your signature and payment  to the address above. There is a minimum three week processing time for transcript requests. Transcripts will be sent by registered courier service from Italy.
